Our Palliative Care Team

Our multidisciplinary team consists of a specially trained physician, registered nurse, social workers and chaplains. Our team works with the primary care physician to minimize the patient’s fears about his/her loss of independence and concerns about becoming a burden to their families and loved ones.

We provide comfort and support while helping our patients and their families create realistic goals of care and help with decision making throughout their stay at P/SL. We also can recommend that they be followed by a palliative care program once they leave our facility.

The P/SL Palliative Care Program is a member of the National Palliative Care Registry.
