Patient and Family Involvement

At HealthONE hospitals, we recognize that patients and families can help improve safety. To encourage patient and family participation during a hospital stay, we provide information on how they can take an active role safety by asking questions, raising concerns and providing complete, accurate information about prescription medications that the patient has been on prior to the hospital admission. One of our hospitals, Sky Ridge Medical Center, has even produced a video that can be viewed on demand in patient rooms to encourage their participation.

Multidisciplinary Rounds

In the intensive care units at HealthONE hospitals a team of physicians, nurses and other key caregivers to- gether create a daily care plan for each patient. The plan includes short-term goals for the patient, which are then shared with the patient and family.

Teamwork Communication & Training

Breakdowns in communication can and do lead to patient harm. For this reason, HealthONE hospitals practice an evidence-based teamwork system that is designed to improve communication and skills that benefit patient care. The communication technique we follow is called SBAR.

SBAR originated in the U.S. Navy submarine community to quickly provide critical information. It is a standardized framework for sharing information. This framework requires four essential elements in every conversation about patient care:

Situation: What is happening with the patient?

Background: What is the clinical background?

Assessment: What do you think the problem is?

Recommendation/Request: What would I recommend or do to correct it?

The communication technique we follow is called SBAR — Situation, Background, Assessment, Recom- mendation. SBAR originated in the U.S. Navy submarine community to quickly provide critical information. It is a standardized framework for sharing information. We also incorporate TEAMSTEPPS, a training tool developed by the Department of Defense for teams to improve commu- nications among each other.
